How to create a custom form in Windows NT 4.0

A.    Custom forms allow the user to create form names for non-standard 
      media sizes.  The form provides entries for defining media sizes 
      (width and height) and margins (left, right, top and bottom).
           
B.    To create additional forms, the user must have Full Control access 
      permissions. Members of the Administrators, Server Operators, Print
      Operators, or Power Users group have Full Control permission by 
      default.  If the user has the correct permission, perform the 
      following steps to create a custom form:
    
      1.    Click on the START button.

      2.    Click on SETTINGS.

      3.    From the SETTINGS list, click on PRINTERS.  

      4.    The Printers window is displayed.  It is not necessary
            to select a specific printer driver, since changing/adding a 
            form will affect all printers on the print server.
            
      5.    From the FILE pulldown menu, select SERVER PROPERTIES.

      6.    Click on the FORMS tab of the Print Server Properties window.
                 
      7.    To create a new form, perform the following:

            a.    Click on the check box next to "Create a New Form". 

            b.    Enter a descriptive title next to "Form Description for:"

            c.    Edit the measurements in the lower portion of the window
                  for media size and margins.

            d.    When finished, click on OK.

      8.    To edit an existing form, perform the following:

            a.    Ensure that the check box next to "Create a New Form" 
                  is not checked. 

            b.    Select from the list of forms in the scroll-down menu by
                  clicking on the desired form name.

            c.    View the measurements in the lower portion of the window. 
                  Edit the values, as needed.

            d.    When finished, click on OK.
